I had the same problem with my router. The way to resolve it is to map TCP port 6699 and UDP port 6257 from the public domain to the local computer. You could use other ports, but those are the ones WinMX uses by default. You can verify your WinMX settings by going to settings-->internet connection and then checking that both "incoming TCP connections" and "in/out UDP packets" match the ports on your router.
The reason why you have to do this is because your local computer has a different IP address than your internet IP address, so the router has to map the individual ports in order to connect them. The router does not restrict you for outgoing but only for incoming connections, by default. Your local IP address can be found by running winipcfg, while your internet address can be found here:

Here is a screenshot of my router settings. As you can see, I have two computers hooked up two it, one with local IP address 192.168.254.110 and the other with address 192.168.254.153. The one with 153 as the last digits is also set up for WinMX, but the ports are different from the default there (or otherwise I wouldn't be able to have set it up for both computers) so I also had to make the same matching adjustments in WinMX on that computer.

doran, your post reminds me of one I made June 2, 2002, but mine was more useful because it had the screenshot, so you could have just linked to it here:
